EFFECT OF PROBIOTIC Lactobacillus paracasei ON TUMOR NECROSIS FACTORALPHA LEVEL IN GINGIVAL CREVICULAR FLUID AMONG PATIENTSUNDERGOING IMPACTED THIRD MOLAR REMOVAL

Abstract

Objective: To evaluate the effects of the probiotic Lactobacillus paracasei on Tumor Necrosis Factor Alpha (TNF- α) levels in gingival crevicular fluid, pain, swelling, trismus, and interincisal distance among patients undergoing impacted third molar removal. Materials and Methods: The population consisted of 30 patients who were undergoing impacted third molar removal at the Department of Oral Surgery and Oral Medicine at Srinakharinwirot University. They divided into two groups: wound irrigation with a probiotic and with 0.9% normal saline for the control group. Also, gingival crevicular fluid was collected. The subjective visual analog scale score of pain, swelling, trismus, and interincisal distance were collected at three different time periods (baseline before surgery, 24 hours, and seven days after the operation respectively). The gingival crevicular fluid was determined using the TNF- α level by ELISA technique. The data were statistically analyzed with inferential statistics (ANCOVA and Mann-Whitney U test) at a statistically significant level of p<0.05 significantly. Result: The probiotic Lactobacillus paracasei -MSMC39 could reduce the TNF-α level in gingival crevicular fluid in the experimental group and had a higher percentage of TNF-α inhibition than the control group (p<0.05). The visual analog scale score of pain, swelling, and trismus in the experimental group was lower than in the control group (p>0.05). Meanwhile, the result indicated that the mean of the interincisal distance of the experimental group was higher than the mean of the control group at a statistically significant level of p=0.014. Conclusion: The results indicated that probiotic Lactobacillus paracasei can reduce TNF-α levels in gingival crevicular fluid and had positive effects on the reduction of post-operative complications in the terms of pain, swelling, and trismus.

วัตถุประสงค์ของงานวิจัย : เพื่อศึกษาผลของโพรไบโอติก Lactobacillus paracasei ที่มีต่อระดับของไซโตไคน์ที่ทำให้เกิดการอักเสบชนิด TNF-α ในน้ำเหลืองเหงือก อาการปวด อาการบวม ภาวะการอ้าปากลำบาก และระยะการอ้าปากในผู้ป่วยที่ได้รับการผ่าฟันคุด วัสดุและวิธีการทดลอง : เก็บข้อมูลจากผู้ป่วยที่เข้ามารับการผ่าฟันคุดล่าง ที่ภาควิชาศัลยศาสตร์และเวชศาสตร์ช่องปาก คณะทันตแพทยศาสตร์ มหาวิทยาลัยศรีนครินทรวิโรฒ จำนวน 30 คน โดยแบ่งเป็นสองกลุ่มคือ (1) กลุ่มทดลองที่ได้รับการล้างแผลด้วยน้ำเกลือผสมโพรไบโอติก และ (2) กลุ่มควบคุมที่ได้รับการล้างแผลด้วยน้ำเกลือ 0.9% โดยมีการเก็บรวบรวมข้อมูลน้ำเหลืองเหงือก ข้อมูลอาการปวด อาการบวม การอ้าปากลำบากด้วย Visual analog scale score และระยะการอ้าปาก ทั้งหมด 3 ครั้ง คือก่อนการทำหัตถการ หลังการทำหัตถการที่ 24 ชั่วโมง และ หลังการทำหัตถการ 7 วัน ซึ่งน้ำเหลืองเหงือกจะมีการนำมาวิเคราะห์หาปริมาณระดับของ TNF-α ด้วยวิธีการ ELISA โดยข้อมูลจะมีการนำมาวิเคราะห์ทางสถิติด้วยสถิติอ้างอิง (ANCOVA และ Mann-Whitney U test) ที่ระดับความเชื่อมั่น p<0.05 ผลการทดลอง : โพรไบโอติก Lactobacillus paracasei -MSMC39 สามารถลดระดับการหลั่งของสารสื่ออักเสบชนิด TNF-α ในน้ำเหลืองเหงือกในกลุ่มทดลอง และมี %TNF-α inbition ที่ดีกว่ากลุ่มควบคุม (p<0.05) สำหรับ Visual analog scale score ของอาการปวด อาการบวม และภาวะการอ้าปากลำบากในกลุ่มทดลองนั้นมีค่าน้อยกว่าในกลุ่มควบคุม (p>0.05) ในขณะที่ผลการทดลองแสดงให้เห็นว่าค่าเฉลี่ยของระยะการอ้าปากของกลุ่มทดลองมีค่าสูงกว่าค่าเฉลี่ยของระยะการอ้าปากของกลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติที่ p=0.014 สรุปผลการทดลอง : ผลการวิจัยนี้ชี้ให้เห็นว่าโพรไบโอติก Lactobacillus paracasei สามารถลดระดับของสารสื่ออักเสบ TNF-α ในน้ำเหลืองเหงือกได้ และแสดงให้เห็นถึงผลเชิงบวกของการลดภาวะแทรกซ้อน อาการปวก อาการบวม และภาวะการอ้าปากลำบาก ที่เกิดขึ้นภายหลังการทำหัตถการ
