RESEARCH AND DEVELOPMENT TEACHING PROGRAM FOR ORAL HEALTH LITERACY AND PATIENT-CENTERED COMMUNICATION PROMOTION OF DENTAL STUDENT

Abstract

The objectives of this research and development study were as follows: 1) to develop a teaching program for oral health literacy and patient-centered communication promotion among dental students; and 2) to study the effectiveness of a developed teaching program to promote oral health literacy and patient-centered communication among dental students. The instruments used in this study consisted of oral health literacy and patient-centered communication self-evaluation questionnaire consisting of knowledge-based multiple choice questions about oral health and performance assessment. The reliability of each measurement was between .974 to .980. A total of sixty dental students participated in this study; thirty dental students in experiment group and thirty dental students in the control group. The data were analyzed by descriptive statistics, basic statistics, MANOVA and ANOVA with repeated measures. The results in objective two found that in time the first, the dental students who particpated in the experiment a group had a mean score higher than the control group immediately following the intervention phase (Pillai's Trace = 0.843, F=152.858, P<.05) and a developed teaching program had a small effect size in oral health literacy (ω2 = .256) and patient-centered communication behavior (ω2 = .306). The scores of knowledge-based MCQ in immediately after intervention and in the four week follow up phase which had interaction between the experimental group and the time of measurement (F = 14.266, 6.316, ω2 = .791, .912, P<.05, respectively).

การศึกษาครั้งนี้เป็นการวิจัยและพัฒนา มีจุดมุ่งหมาย 1) เพื่อสร้างและออกแบบโปรแกรมการจัดการเรียนรู้เพื่อการส่งเสริมความรอบรู้ด้านสุขภาพช่องปากและพฤติกรรมการสื่อสารที่เน้นผู้ป่วยเป็นศูนย์กลางจากการทบทวนวรรณกรรมอย่างเป็นระบบ 2) เพื่อทดสอบประสิทธิผลของโปรแกรมการจัดการเรียนรู้เพื่อส่งเสริมความรอบรู้ด้านสุขภาพช่องปากและพฤติกรรมการสื่อสารที่เน้นผู้ป่วยเป็นศูนย์กลางของนิสิตทันตแพทย์ เก็บข้อมูลจากแบบวัดความรอบรู้ด้านสุขภาพช่องปาก แบบวัดพฤติกรรมการสื่อสารที่เน้นผู้ป่วยเป็นศูนย์กลาง แบบทดสอบความรู้พื้นฐานเกี่ยวกับสุขภาพช่องปาก และแบบประเมินการทดสอบปฏิบัติกับผู้ป่วยจำลอง ที่มีค่าความเชื่อมั่นเท่ากับ .980, .974, .944, และ .938 ตามลำดับ กลุ่มตัวอย่าง ได้แก่ นิสิตทันตแพทย์ 60 คน เป็นกลุ่มทดลองและกลุ่มควบคุมกลุ่มละ 30 คน วิเคราะห์ข้อมูลด้วยการวิเคราะห์เชิงเนื้อหา สถิติพื้นฐาน สถิติความแปรปรวนหลายตัวแปร (MANOVA) และการวิเคราะห์ความแปรปรวนแบบวัดซ้ำ (Repeated ANOVA) และพบผลการทดสอบโปรแกรมฯ พบว่า นิสิตทันตแพทย์ในกลุ่มทดลองที่ได้รับโปรแกรมการจัดการเรียนรู้มีคะแนนเฉลี่ยของความรอบรู้ด้านสุขภาพช่องปากและพฤติกรรมการสื่อสารที่เน้นผู้ป่วยเป็นศูนย์กลางสูงกว่ากลุ่มควบคุมที่ไม่ได้รับโปรแกรมฯ ในระยะวัดผลหลังการทดลองทันที (Pillai's Trace = 0.843, F = 152.858, P<.05) ซึ่งโปรแกรมฯ มีขนาดอิทธิพลอยู่ในระดับมากที่มีต่อความรอบรู้ด้านสุขภาพช่องปาก (ω2 = .256) และพฤติกรรมการสื่อสารที่เน้นผู้ป่วยเป็นศูนย์กลาง (ω2 = .306) 2) ผลการวิเคราะห์คะแนนความรอบรู้ด้านสุขภาพช่องปากและพฤติกรรมการสื่อสารที่เน้นผู้ป่วยเป็นศูนย์กลางด้วยการวิเคราะห์ความแปรปรวนหลายตัวแปรในระยะวัดผลหลังการทดลองทันทีและหลังการทดลอง 4 สัปดาห์ พบว่า มีปฏิสัมพันธ์ร่วมกันระหว่างกลุ่มการทดลองและครั้งของการวัดของตัวแปรความรอบรู้ด้านสุขภาพช่องปากและพฤติกรรมการสื่อสารที่เน้นผู้ป่วยเป็นศูนย์กลาง (F = 14.266, 6.316, ω2 = .791, .912, P<.05 ตามลำดับ)
