Title: A LONG-TERM EFFECT OF WEARING A REUSABLE MASKWHILE RUNNING EXERCISE ON BLOOD GAS LEVELS IN ADULTS

Abstract: Background: There is the problem around the world regarding pandemic diseases of the respiratory system caused by viruses, such as COVID-19 and tuberculosis (TB), and the problems of air pollution, small dust, and PM2.5 leading to wearing masks in daily life, including running exercise. Objectives: To study and compare the long-term effects of wearing reusable masks on blood gas (PaO2, PaCO2, pH, HCO3) and hematocrit (HCT) in moderate and vigorous intensity running exercise in groups of adults. Methods: there were 15 healthy volunteers, aged 25-40 years, no underlying disease, and experienced in running exercises were randomized into two groups: moderate and a vigorous intensity groups. Both groups wore reusable masks which could filter the air at 95% during running exercises for six weeks. Results: Carbon dioxide in arterial blood (PaCO2) and bicarbonate in the red blood (HCO3) significantly increased (p < 0.05) after wearing reusable masks in running exercise. There were no significant differences observed in the other variables. The comparison between the two groups found that the amount of bicarbonate in the arterial blood (HCO3) in the vigorous-group was significantly increased more than moderate-group (p < 0.01), but there were no significant differences between the groups were observed in other variables. Conclusions: Wearing a mask while running for a long time influenced bodies associated with Primary Respiratory Acidosis. However, the body retains the ability to reduce acidity in the blood (pH) without increasing the number of red blood cells. Instead, rectification was performed through the proximal ureter using bicarbonate (HCO3) as a chemical substrate. As a result, there was a decrease in the amount of water in the blood, which was associated with the occurrence of the state of thick blood (Relative polycythemia).
ที่มา ทั่วโลกได้เผชิญปัญหาของโรคระบาดระบบทางเดินหายใจที่มีสาเหตุทั้งจากไวรัส เช่น โรคโควิด19 (COVID19) และแบคทีเรีย เช่น วัณโรค (Tuberculosis : TB) รวมถึงปัญหามลพิษทางอากาศ ฝุ่นละอองขนาดเล็ก และ PM2.5 จนนำไปสู่การใส่หน้ากากอนามัยในชีวิตประจำวันร่วมถึงการวิ่งออกกำลังกาย วัตถุประสงค์ เพื่อศึกษาและเปรียบเทียบผลระยะยาวของการใส่หน้ากากอนามัยแบบใช้ซ้ำที่มีต่อค่าแก๊สในเลือด (PaO2 PaCO2 pH HCO3) และค่าฮีมาโทคริต (HCT) ในขณะวิ่งออกกำลังกายที่ความหนักปานกลางและความหนักสูงของคนวัยทำงาน วิธีการวิจัย อาสาสมัครจำนวน 15 ราย ที่มีสุขภาพดี อายุระหว่าง 25 – 40 ปี  ไม่มีโรคประจำตัว และมีประสบการณ์ในการวิ่งออกกำลังกาย ถูกสุ่มแบ่งออกเป็น 2 กลุ่ม คือ กลุ่มความหนักปานกลางและกลุ่มความหนักสูง ทั้ง 2 กลุ่มใส่หน้ากากอนามัยแบบใช้ซ้ำมีความสามารถในการกรองอนุภาคที่ 95% ขณะวิ่งออกกำลังกายเป็นระยะเวลา 6 สัปดาห์ ผลการศึกษา หลังใส่หน้ากากวิ่งออกกำลังกาย ค่าคาร์บอนไดออกไซด์ (PaCO2) และค่าไบคาร์บอเนต (HCO3) ในเลือดแดงมีปริมาณเพิ่มสูงขึ้นอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ไม่พบการเปลี่ยนแปลงอย่างมีนัยสำคัญต่อตัวแปรอื่น ๆ  ในการเปรียบเทียบระหว่าง 2 กลุ่มพบว่า ค่าไบคาร์บอเนต (HCO3) ในเลือดแดงของกลุ่มความหนักสูงมีปริมาณเพิ่มสูงขึ้นมากกว่ากลุ่มความหนักปานกลางอย่างมีนัยสำคัญทางสถิติที่ระดับ .01 แต่ไม่พบความแตกต่างอย่างมีนัยสำคัญในตัวแปรอื่นเมื่อเปรียบเทียบระหว่างกลุ่ม สรุปผล การใส่หน้ากากอนามัยวิ่งออกกำลังกายระยะยาวส่งผลให้ร่างกายมีความสัมพันธ์กับการเกิดสภาวะ Primary Respiratory Acidosis แต่ร่างกายยังคงความสามารถในการปรับลดสภาวะความเป็นกรดในเลือด (pH) โดยไม่มีการเพิ่มจำนวนของเม็ดเลือดแดง แต่ใช้การปรับแก้ผ่านท่อไตส่วนต้นโดยใช้ไบคาร์บอเนต (HCO3) เป็นเคมีตั้งต้น ส่งผลให้มีการลดลงของปริมาณน้ำในเลือดทำให้เกิดความสัมพันธ์กับการเกิดสภาวะเลือดข้น (Relative polycythemia)
