Abstract: According to the estimation of the WHO, 80% of the population with knee OA had mobility limitations, and 25% of individuals with knee OA were unable to perform daily activities. However, there is no data to determine how much the proportion of knee OA patients with mobility limitations and decreased functional lower extremity strength based on physical performance test screening. The aim of this study was to define the number and percentage of knee OA patients with mobility limitations and decreased functional lower extremity strength by a 30-second chair stand test (30s-CST). The population consisted of 40 knee OA patients, aged 50–69 years, were recruited by following the criteria of the American College of Rheumatology (ACR). The numeric rating scale of all patients were greater than four. They were assessed for physical performance by 30s-CST. If they performed 30s-CST less than 12 repetitions, it means that knee OA patients had mobility limitations and decreased functional lower extremity strength. Descriptive statistics were used to determine the number and percent of knee OA patients with mobility limitations and decreased functional lower extremity strength. The results of knee OA patients who were recruited in this study performed 30s-CST less than 12 repetitions. One hundred percent of knee OA patients showed mobility limitations and decreased functional lower extremity strength. The mean repetitions of 30sCST in knee OA patients, aged 50-69 years, were 7.95±1.78 repetitions. In conclusion, all patients with knee OA exhibited mobility limitations and decreased functional lower extremity strength. They were able to perform a full stand for approximately eight repetitions in 30 seconds, which was less than the mean number of full stands for the elderly. It was inferred that all knee OA patients showed greater mobility limitations and decreased functional lower extremity strength when compared to the elderly. The 30s CST can be used to clinically screen mobility and functional lower extremity strength in knee OA patients. Abstract: The study aimed to assess fall risk factor regarding leg muscle strength and walking balance in the young elderly of the Sao Thong Hin subdistrict municipality, and to analyze the relation between the number of young elders who had falling risk when assessed by Timed up and go test (TUGT) and by Five Times sit to stand test (FTSST). This cross-sectional study was conducted in 134 young elderly volunteers aged 60–69 years of the Sao Thong Hin subdistrict municipality, Nonthaburi Province. The elderly volunteers were assessed fall risk factors in terms of leg muscle strength by using FTSST and walking balance by using TUGT. Results showed that an average age of 134 young elderly volunteers was 64.08 ± 2.66 years. When considering fall risk factor regarding leg muscle strength which assessed by FTSST, there were 36 people (26.9%) who were at risk of falling. Meanwhile, there were 10 people (7.5%) who had a fall risk factor regarding walking balance assessed by TUGT. Within the elderly who had fall risk when assessed by either FTSST or TUGT, there were 7 people (5.2%) who are affected by both factors. Additionally, it was found that number of the elderly who had fall risk assessing by FTSST and by TUGT were significantly related, X2 (1, N = 134) = 10.23, p = 0.001. In conclusion, number of the young elderly in Sao Thong Hin subdistrict municipality who had fall-risk factor regarding leg muscle strength was more than those who had fall-risk factor regarding walking balance. It was also found that screening of falling risk by TUGT was significantly associated with those by FTSST. Thus, the elderly in the Sao Thong Hin subdistrict municipality should receive both strengthening and walking balance training to promote their physical performance since they are young elders in order to reduce falling risk from age-related deterioration of leg muscle strength and walking balance.; การศึกษานี้มีวัตถุประสงค์เพื่อประเมินปัจจัยเสี่ยงล้มด้านความแข็งแรงของกล้ามเนื้อขาด้วย Five times sit to stand test (FTSST) และด้านการทรงตัวขณะเดินด้วย Timed up and go test (TUGT) ของผู้สูงอายุวัยต้นในเขตเทศบาลตำบลเสาธงหิน และความสัมพันธ์ของจำนวนผู้สูงอายุวัยต้นที่เสี่ยงล้มเมื่อคัดกรองด้วย FTSST และ TUGT โดยดำเนินการศึกษาแบบภาคตัดขวางในอาสาสมัครผู้สูงอายุวัยต้น อายุ 60-69 ปี จำนวน 134 คน ในเขตพื้นที่เทศบาลตำบลเสาธงหิน จังหวัดนนทบุรี ผู้สูงอายุได้รับการประเมินปัจจัยเสี่ยงล้มด้านความสามารถในการทรงตัวด้วย TUGT และประเมินปัจจัยเสี่ยงล้มด้านความแข็งแรงกล้ามเนื้อขาด้วย FTSST จากนั้นวิเคราะห์และรายงานข้อมูลที่ได้ด้วยสถิติเชิงพรรณนาและทดสอบความสัมพันธ์ของจำนวนผู้สูงอายุวัยต้นที่เสี่ยงล้มระหว่างการคัดกรองด้วย FTSST และ TUGT ด้วย Chi-square test โดยกำหนดนัยสำคัญที่ p< 0.05 ผลการศึกษาพบผู้สูงอายุวัยต้น ที่เข้าร่วม จำนวน 134 คน มีอายุเฉลี่ย 64.08 ± 2.66 ปี มีปัจจัยเสี่ยงล้มด้านความแข็งแรงกล้ามเนื้อขาเมื่อประเมินด้วย FTSST จำนวน 36 คน (26.9 %) และมีปัจจัยเสี่ยงล้มด้านความสามารถในการทรงตัวเมื่อประเมินด้วย TUGT จำนวน 10 คน (7.5 %) โดยเป็นผู้ที่เสี่ยงล้มจากปัจจัยทั้งสองด้านร่วมกันอยู่ 7 คน (5.2 %) และพบว่าการคัดกรองปัจจัยเสี่ยงล้มด้านความสามารถการทรงตัวด้วย TUGT มีความสัมพันธ์กับการคัดกรองปัจจัยเสี่ยงล้มด้านความแข็งแรงของกล้ามเนื้อขาด้วย FTSST อย่างมีนัยสำคัญทางสถิติ X2 (1, N = 134) = 10.23, p = 0.001 สรุปได้ว่า ผู้สูงอายุวัยต้น ในเขตเทศบาลตำบลเสาธงหิน มีปัจจัยเสี่ยงล้มด้านความแข็งแรงของกล้ามเนื้อขามากกว่าปัจจัยด้านความสามารถในการทรงตัว และการประเมินการเสี่ยงล้มด้วย FTSST และ TUGT มีความสัมพันธ์กัน ดังนั้น ควรให้การส่งเสริมสมรรถภาพร่างกายของผู้สูงอายุในเขตเทศบาลเสาธงหินทั้งด้านความแข็งของกล้ามเนื้อขาและการทรงตัวขณะเดินตั้งแต่วัยต้นเพื่อลดความเสี่ยงล้มจากปัจจัยเสี่ยงล้มดังกล่าว0015-01-01T00:00:00ZVALIDITY OF THE STROKE-BALANCE EVALUATION SYSTEMS TEST

(S-BESTEST) IN PEOPLE WITH CHRONIC STROKE; ความเที่ยงตรงของแบบประเมิน Stroke-Balance Evaluation Systems Test
(S-BESTest) ในผู้ป่วยโรคหลอดเลือดสมองระยะเรื้อรัง
Abstract: The Stroke-Balance Evaluation Systems Test (S-BESTest) is a reliable and valid system-based assessment of balance impairments in patients with subacute strokes. Patients with chronic and acute strokes have different movement and balance characteristics, according to rates of neuromuscular recovery, secondary complications, and the use of compensatory strategies. However, the S-BESTest has not been validated among people with chronic strokes. Therefore, this study aimed to examine the concurrent validity of the S-BESTest in people with chronic strokes. The method included rater training and reliability testing, which were performed before the validity test. Then, three physical therapists examined the performances of the patients from the same set of ten video clips on two separate occasions, performed 10 days after the first occasion. The scores from the first and second occasions were used to determine intra-rater reliability using the intraclass correlation coefficient (ICC) model 3.1. There were 20 participants with chronic strokes assessed with balance assessment tools including the S-BESTest, the Berg Balance Scale (BBS), and the Community Balance and Mobility Scale (CB&M). Concurrent validity was assessed with the BBS and the CB&M using Pearson's Product-Moment Correlation. The statistical significance was set at a p-value of <0.05. The ceiling and floor effects were calculated by percentage, frequency of the lowest or highest possible scores achieved by participants. The results were as follows: the intra-rater reliability of the total and domain scores of the S-BESTest were excellent with an ICC (95% CI) of 0.96-0.99 (0.85-0.99) and 0.93-1.00 (0.71-1.00), respectively. The S-BESTest had an excellent correlation with the BBS (r = 0.93, p<0.01) and the CB&M (r = 0.86, p<0.01). The results showed no floor and ceiling effects of the S-BESTest in people with chronic strokes, while the BBS showed the trend of the floor effect (10%) and 15% of participants had very low scores on the CB&M. In conclusion, the S-BESTest was reliable, but clear instructions of how to score. Rater practice and discussing the session with an experienced physical therapist are necessary for such a degree of the reliability of the S-BESTest. The S-BESTest was valid in assessing balance problems and the test items were challenging enough, but not too difficult for patients with chronic strokes who live in the community.; ผู้ป่วยโรคหลอดเลือดสมองในแต่ละระยะ (ระยะเฉียบพลัน ถึง ระยะเรื้อรัง) จะมีลักษณะจำเพาะที่แตกต่างกัน เนื่องจากระยะเวลาการฟื้นตัวและภาวะแทรกซ้อน โดยแบบประเมิน Stroke-Balance Evaluation Systems Test (S-BESTest) เป็นแบบประเมินความบกพร่องของระบบที่ส่งผลต่อการทรงตัว จากศึกษาที่ผ่านมา พบว่าเป็นแบบประเมินที่มีความเที่ยงและความตรง สำหรับการนำไปประเมินในผู้ป่วยโรคหลอดเลือดสมองระยะกึ่งเฉียบพลัน แต่อย่างไรก็ตาม ยังไม่มีการศึกษาความเที่ยงของแบบประเมิน S-BESTest สำหรับการนำไปใช้ในผู้ป่วยโรคหลอดเลือดสมองระยะเรื้อรัง ดังนั้นวัตถุประสงค์ของการศึกษานี้ เพื่อทดสอบความตรงเชิงสภาพ (concurrent validity) ของแบบประเมิน S-BESTest ในผู้ป่วยโรคหลอดเลือดสมองระยะเรื้อรัง โดยก่อนการทดสอบความตรง จะมีการฝึกใช้แบบประเมินและทดสอบความเที่ยงของผู้วัดจำนวน 3 ท่าน ผ่านวิดีโอประเมินการทรงตัวของอาสาสมัคร 10 คน จำนวน 2 รอบ ระยะเวลาในการทดสอบแต่ละรอบห่างกัน 10 วัน และนำคะแนนประเมินทั้ง 2 รอบ มาวิเคราะห์โดยใช้สถิติ intraclass correlation coefficient (ICC) model 3,1 จากนั้นจึงทำการทดสอบความตรงของแบบประเมิน S-BESTest แบบประเมิน Berg Balance Scale (BBS) และแบบประเมิน the Community balance and mobility scale (CB&M) ในอาสาสมัครจำนวน 20 คน โดยใช้สถิติ Pearson's Product-Moment Correlation กำหนดนัยสำคัญทางสถิติที่ p-value <0.05 และคำนวณค่า Ceiling-Floor effects โดยใช้ร้อยละความถี่ของคะแนนต่ำสุดและสูงสุดที่เป็นไปได้ของอาสาสมัคร จากการศึกษา พบว่าทั้งคะแนนรวมและแต่ละโดเมนของแบบประเมิน S-BESTest มีความเที่ยงภายในผู้วัดสูง (ICC (95% CI) = 0.96-0.99 (0.85-0.99) และ 0.93-1.00 (0.71-1.00) ตามลำดับ) และพบว่าแบบประเมิน S-BESTest มีความสัมพันธ์กับแบบประเมิน BBS (r = 0.93, p<0.01) และแบบประเมิน CB&M (r = 0.86, p<0.01) อีกทั้งยังไม่พบคะแนนต่ำสุดและสูงสุดที่เป็นไปได้ของแบบประเมิน S-BESTest ในผู้ป่วยโรคหลอดเลือดสมองระยะเรื้อรัง ในขณะที่แบบประเมิน BBS พบว่ามีแนวโน้มของคะแนนต่ำสุด (ร้อยละ 10) และร้อยละ 15 ของอาสาสมัครที่มีคะแนนแบบประเมิน CB&M ค่อนข้างต่ำ จากผลการศึกษาสรุปได้ว่าแบบประเมิน S-BESTest เป็นแบบประเมินที่มีความเที่ยง และขั้นตอนการฝึกใช้แบบประเมินก่อนทดสอบความเที่ยง ร่วมกับนักกายภาพบำบัดที่มีประสบการณ์การใช้แบบประเมินดังกล่าว เป็นขั้นตอนที่จำเป็น มีผลต่อระดับความเที่ยงของแบบประเมิน อีกทั้งแบบประเมิน S-BESTest ยังมีความตรง สำหรับประเมินปัญหาการทรงตัว และมีข้อทดสอบที่ท้าทายเพียงพอ แต่ไม่ยากเกินไปสำหรับผู้ป่วยโรคหลอดเลือดสมองระยะเรื้อรังที่อาศัยอยู่ในชุมชน0017-01-01T00:00:00ZIDENTIFICATION OF POSTURAL CONTROL SYSTEM IMPAIRMENTS IN OLDER ADULTS WITH CHRONIC NECK PAIN

Title: IDENTIFICATION OF POSTURAL CONTROL SYSTEM IMPAIRMENTS IN OLDER ADULTS WITH CHRONIC NECK PAIN; การระบุความบกพร่องด้านการทรงตัวในผู้สูงอายุที่มีปัญหาปวดคอเรื้อรัง
Abstract: Chronic neck pain (CNP) can affect postural control among older adults. Gait performance, in terms of gait speed and symmetry, is an indicator of dynamic postural control. The Balance Evaluation Systems Test (BESTest) is used to assess the systems of postural control. This study aims to determine which gait variables were more sensitive to dynamic postural control among older adults with CNP and assessed that BESTest could identify postural control impairments in CNP and item difficulty by Rasch analysis. This cross-sectional study recruited 30 young adults (YOUNG), aged 20-40 years, and 80 older adults, aged 60 years or older [without neck pain (OLD) = 60, with chronic neck pain (CNP) = 20]. Questionnaires were used to collect demographic data, neck pain intensity, self-rated neck pain and disability, and balance confidence. The 10-Meter Walk test was used to assess gait performance with three different conditions: no head movement (NM), horizontal head movement (HM), and vertical head movement (VM). BESTest was used to assess postural control. The Inertial Measurement Unit was used to capture and analyze gait parameters. The CNP group reported moderate pain and zero to mild disability in daily activities. Balance confidence was moderately correlated with gait speed (r=0.62) and moderately inversely correlated with gait asymmetry during HM and VM (-0.56 and -0.69, respectively). The CNP group showed slower gait speed during HM and VM (p<0.05), lower stride length and cadence. The gait asymmetry index in the CNP group was higher than the OLD group in VM (p<0.05). Thus, gait speed with a head movement was more sensitive. Compared to the YOUNG group, the BESTest score was lower in the OLD group, the CNP group had the lowest score, in biomechanical constraints, transitions-anticipatory postural adjustment and reactive postural response (p< 0.05). The BESTest was the most accurate of the three was 48.5 of 51 for older adults whose daily life were affected by neck problems with a high AUC (0.79), sensitivity (72%), and specificity (69%). The Timed Up and Go was the most difficult BESTest item for all groups, while 14 items had more difficulty for the CNP group. BESTest can identify postural control impairments in older adults affected by CNP and help clinicians consider management to prevent falls in CNP.; -0021-01-01T00:00:00Z
